The Ministry of Health would like to inform the public that the application form for the issuance of a medical certificate for Public Health reasons has been uploaded on the Press and Information Office’s website on coronavirus and it can be accessed from the following links: https://bit.ly/2wctwgh (Greek) και https://bit.ly/397nJGa (English).

The certificate is issued to persons who must remain in self-confinement for a period of 14 days since their return from a trip abroad or their contact with a confirmed case of coronavirus.

It is noted that along with the application, travel documents (e.g. boarding pass or airline ticket), must be attached to certify the date of arrival to Cyprus.

The signed applications along with the required travel documents should be sent by fax to 22 771496 or email to smichael@papd.mof.gov.cy.
